In the days, weeks, and months following your procedure, your scalp will gradually heal while your newly transplanted follicles establish themselves and begin producing healthy, naturally growing, long-lasting hair. Understanding what to expect during the recovery process—including normal healing, temporary shedding, and the timeline for new growth—can help you feel more confident and ensure you achieve the best possible results from your investment.

To anticipate a common question his patients ask, there is no significant difference in how transplanted hair sheds, regrows, or matures following a Follicular Unit Excision (FUE) or Follicular Unit Transplantation (FUT) procedure. The primary difference for the FUT during recovery involves the donor area, as the procedure requires sutures. For this reason, additional temporary activity restrictions may be necessary.

Although the following reflects Dr. Barusco’s post-operative guidance, each patient’s timeline may vary somewhat depending on age, healing characteristics, medical history, and the area transplanted.

Days One Through Three

• Mild swelling of the forehead may occur.
• Small scabs begin forming around the grafts.
• Patients should rest, avoid strenuous activity, and sleep with their head elevated.
• Mild redness and tenderness are expected.

 

Days Four Through Seven

• Swelling typically resolves.
• Scabbing begins to loosen and shed. It is important not to pick at the scabs or scratch your scalp, as doing so may cause the grafts to be damaged or lost.
• Most patients feel comfortable returning to work and normal social activities around seven days post-procedure.
• Patients may begin resuming exercise after approximately one week depending on procedure type, as the FUT may require up to 14 days due to sutures.

Keep in mind: Contact your surgeon if you experience unusual swelling, inflammation or discomfort—or have questions about shampooing techniques or any other matter regarding how you should care for your hair and scalp during this initial time. Following advice you find online—regardless how reputable the source seems—can lead to complications or a poor outcome. Weeks Two Through Eight

• The transplanted hair shafts commonly enter a temporary shedding phase, which is completely normal and expected. Although the visible hairs may shed, the transplanted follicles remain safely established beneath the scalp and will begin producing new growth in the months ahead.
• Mild pinkness may persist in some patients, particularly those with fair skin.

 Months Two and Three

• This can be one of the more emotionally challenging periods for patients, as they may look similar to their pre-operative state. Maintaining patience and a positive attitude during this phase is critical.
• Dormant follicles begin preparing for new growth.
• Some patients notice early fine hairs beginning to emerge.

 Months Three and Four

• Initial regrowth begins.
• New hairs are often fine, lighter in color, and somewhat wiry in texture initially.
• Patients usually begin seeing the first signs that the procedure is working.

 Months Five and Six

• Noticeable cosmetic improvement becomes evident.
• Increased density and coverage develop.
• Hair begins to gain caliber and improve in texture.

Keep in mind: While you might expect your hair to grow in at the same time, don’t be alarmed if it’s somewhat patchy and grows at different rates during this stage. About six months after your hair transplant, you may see that the new growth is not necessarily full or consistent around your head, but be patient. Despite the different growth rates, it usually looks natural throughout this time. In fact, such gradual growth creates the impression that you haven’t had a hair transplant.

Months Six Through Nine

• Significant growth occurs.
• Patients often notice substantial improvements in framing and overall appearance.
• Hair continues to mature and thicken.

 Months Nine Through Twelve

• By this stage, many patients have achieved approximately 80–90% of their anticipated result, with continued improvement in density, texture, and maturity.
• Density, texture, and styling options continue to improve.

 Months Twelve Through Eighteen

• Final maturation phase.
• Hair reaches its maximum caliber, texture, and cosmetic impact.
• Crown procedures in particular may continue improving through the 18-month mark.
